Insurance Information

 

Medicare Patients 

Did you know that  Medicare covers Nutrition Counseling for Diabetes and Chronic Kidney Disease? Call your insurance and ask if they cover nutrition counseling for diabetes or chronic kidney disease. Bring a referral from your doctor to the appointment.

We accept all Medicare Plans, other than Medicare Plus Blue.

Prior to your visit you can check with your insurance company for the Dietary Counseling coverage for following codes: 97802 and 97803.

Aetna Patients 

Aetna usually covers Nutritional counseling for Obesity, Diabetes, and Chronic Kidney Disease. Please call your insurance to confirm.

HSA/FSA 

We might be able to take HSA/FSA credit cards. Please check with your insurance and confirm or call us.

For Physicians

Please fax us a referral for your patient at (248)-292-3867. If you have any questions, please give us a call at 248-953-8927. You can also fax us patients lab work at the given fax number. It will be great if we have labs for HbA1c and Fasting Blood Sugar along with Lipid Panel for Diabetic patients and Renal Panel for CKD patients.
